Preauthorization List and Request Form
If you are a medical provider and would like to obtain preauthorization for a specific non-emergency healthcare service or treatment:
- Review the required list of treatments requiring approval before care is rendered in the "Downloads" box on the right side of this page.
- Submit the Preauthorization Request Form also located in the Downloads box.
- Please fax the request form to 888.777.8272 with supporting clinical documentation.
Electronic Billing Information
As a result of legislative changes under House Bill 7 in 2005, the Texas Department of Insurance now requires that the workers' compensation system comply with an electronic billing system to streamline this paper-laden process.
The Texas Department of Insurance, Division of Workers' Compensation adopted Rules that, effective January 1, 2008, require health providers to begin submitting medical bills to carriers electronically. If a provider has less than 10 employees, and workers' compensation represents less than 10 percent of their business, they are exempt from this requirement.
The TASB Risk Management Fund (Fund) has contracted with Jopari Solutions Inc., a California-based supplier of eBilling and payment solutions for the insurance industry, to manage its eBilling process. Jopari is the link between health care providers and the Fund for electronic submission and remittance of electronic bills and attachments.
