Employee Paid Leave Election Forms

These forms are used when an injured employee elects to receive paid leave in lieu of Temporary Income Benefits for the first week and/or all other weeks of lost time. They are located in the Downloads box on the right of this page.  Depending on your school board policy, you will only use one of these forms. The election forms cannot be completed or sent on line. The injured worker must complete the form and you must send or fax it to your adjuster at:
