Injured Employee Rights & Responsibilities Notice

Note to employers: You must provide this Notice and the First Report of Injury to the injured worker at the same time you report the injury to your insurance carrier. This information must be provided in English and the language common to the worker. Refer to the list of Notices by language below. The Notices may be reproduced or photocopied as necessary but may not be edited or altered in any way (DWC Rule 120.2).

Employee Paid Leave Election Forms

These forms are used when an injured employee elects to receive paid leave in lieu of Temporary Income Benefits for the first week and/or all other weeks of lost time. They are located in the Downloads box on the right of this page.  Depending on your school board policy, you will only use one of these forms. The election forms cannot be completed or sent on line. The injured worker must complete and sign the form, and you must send or fax it to your adjuster at:
