WC Membership Benefits: Online Claims Reporting

Hundreds of districts are already taking advantage of the convenience of electronic claims reporting . The First Report of Injury or Illness form can be completed on line through this Web site. In addition to the First Report of Injury, the Fund has created the Employers' Wage Statement for School Districts (DWC-3SD), Employers' Wage Statement (DWC-3) and Supplemental Report of Injury (DWC-6) forms in Portable Document Format (PDF) which can be both completed and submitted on line to TASB. When the forms are submitted, a copy is also made available for record keeping purposes.  Remember, you must copy the claimant with all claim forms.

As an added benefit, the the Supplemental Report of Injury form (DWC-6) comes with the Post Injury Earnings Worksheet and reporting instructions. This worksheet allows a member to report sick and annual leave, modified duty wages and other post injury payments more efficiently and timely to the Fund to avoid costly overpayments of benefits.
