Update Your Workers’ Compensation Documents!

With all the legislative and state rule changes over the years, it is easy to lose track of the current and proper notices and forms for claim reporting. Below is a short list of what you should update and where to find what you need.
Injury, Work Status, and Wage Reporting
Below are the three main forms an employer must file on a regular basis for WC claims.
- The First Report of Injury IA-1 is required no later than 8 days after an injured worker incurs more than one day of lost time, dies, or reports an occupational illness. This is an online form located on the Fund Website. If you are not a member of the Fund's WC program, your insurance carrier may require the DWC-1.
- The Supplemental Report of Injury − DWC-6 is required to report a return to work, change in earnings (includes sick leave payments while receiving Temporary Income Benefits), resigns, or is terminated. You can find this form on the Fund Website or on the Division of Workers’ Compensation (DWC) Website.
- Wage Statements (DWC-3SD and DWC-3) are required when the injured worker reaches 8 days of disability and when the injured worker receives an impairment rating from a doctor. These forms can also be found on the Fund Website or the DWC Website.
- Paid Leave Election forms are also located on the Fund Website. TASB HR Services has created samples for you to download. Look in the Downloads box to print out Word versions of these forms.
Don’t Forget about Required Postings!
The Employee Rights and Responsibilities form is located on the Office of Injured Employee Council Website. Other required Notices such as the Notice 6 and 9 are located on the DWC Website.
Fund members can also access the Workers’ Compensation Manual behind secured access in myTASB with a user id and password.
Make sure you bookmark the Web sites above for future reference. A good rule of thumb is to check them monthly, especially in a legislative year such as this one.
