  1. Submit a complete description of the risk management initiative, product, or program being submitted.    
  2. Describe the circumstances that led to the development of this initiative, product, or program. 
  3. Who participated and how much time did each participant devote to developing this initiative, product, or program? Were outside consultants used?  
  4. Describe how your program can be adapted for use by other Fund members.   
  5. Indicate the cost of the project and include time invested. Identify the value added as a result of this undertaking. Please quantify if possible.    
  6. What makes your approach unique? Explain the concepts, tools, or techniques that were used to implement your product or program.   
  7. List any outstanding achievement awards or grants that you have received related to risk management given by statewide/national organizations in the last several years.
